MINISTRY FORMS GENERAL INSTRUCTIONS

1st Form: Contact information – you’ll enter your contact information, select your director. Click the blue button at the bottom “Next Build Request”.

2nd Form: Building Request Form – complete with details about your event. Click the blue button at the bottom “Next Communication Form”. Note: for examples of room layouts click here to see different layouts.

3rd Form: Communion Request Form - must be approved and submitted 30 days prior to requested announcement dates. Click the blue button at the bottom “Next Copy/Print/Creative”.

4th Form: Copy/Print/Creative - please allow 5 business days for copy/print requests & up to 10 business days for creating a flyer.
When you have completed and necessary forms, click “Submit to Assigned Director.” Your director will receive an email
submission from you. Once your director has given approval, your form will be submitted to the office admins.
Note:

After you enter your contact information and select your director, you can click next to get to the form you need. For instance, if I just need to complete a copy/print request form, I will click “Next…” until I get to that form.

For all forms submissions, the 1st Form “Contact Information” must be completed and director selected.
